Job Description:
Community Served: Diverse patients receiving social work services in Union Square, New York
Brief Description of Proposed Project:
Through AmeriCorps, work with the clinical and mental health care team to outreach to patients who have recently been discharged from the hospital and to provide care managment services for these patients. Navigator would outreach to patients to enroll them in group, and would co-faciliate these groups with the Care Management department. The HealthCorps Navigator will identify patients who need to be brought into care, facilitate referrals, and provide health education.
Full-time HealthCorps members commit to serve for 1,700 hours over an 11 month period – approximately 40 hours/week from late December2012 to November 2013.
Specific Member Responsibilities and Activities:
Using the electronic health record, identify patients who have recently been discharged from the hospital and are in need of outreach and education.
Reach out to patients via phone to arrange care management sessions, in-clinic educational sessions and/or set up appointments with providers.
Work one on one with patients to discuss barriers to receiving recommended care or adherence to medications, etc; and identify ways to remove those barriers by providing follow-up education and support and referring patients and their families to social workers for additional assistance.
Facilitate groups to assist patients with recieving care.
Collect data on the number of patients contacted and the number who receive and keep appointments

What is The Institute for Family Health?
The Institute for Family Health is a nonprofit organization dedicated to improving the quality and availability of family practice services to medically underserved populations. Founded in 1983, the Institute operates 16 full-time and 9 part-time federally funded community health centers throughout Manhattan, the Bronx, and the Mid-Hudson Valley in New York State, including a school-based health center and eight part-time sites that serve the homeless.
In addition to its clinical programs, the Institute trains health professionals through a tri-state faculty development program, engages in community-based participatory research related to racial and ethnic disparities in health outcomes, and works to promote the diversity of the healthcare workforce through its leadership in the New York State Area Health Education Center (AHEC) System.
What is Community HealthCorps?
The Community HealthCorps is a national AmeriCorps program that promotes health care for the underserved while developing future healthcare workers. HealthCorps members engage in a variety of projects to promote access to health care and health information in medically underserved communities. They can also recruit and develop volunteers to support community health centers.
